Abstract

Hair transplant is one of the most wanted and common procedures in the recent times. Follicular unit extraction or FUE is more commonly performed than strip or FUT technique. As all are aware hair transplant is a team effort and will require many medical personnel to get through the procedure. This will make us dependent on our nursing staff and hair transplant technicians. Sometimes these staff can get dissociated from our Centre and we might have difficulties and we might operate in Centres without much knowledge about hair transplant. Hence came the concept of hair transplant on the go. In this concept, only the operating doctor is the most important person in the team, all the other personnel will be trained during the procedure itself and they can keep changing without affecting the hair transplant procedure. Also, the investment needed for this concept is very minimal, the results are good as the procedure is done by the doctor and supervised directly by him. In this presentation, I will be talking about this concept along with the tips and tricks for beginners to start and setup a hair transplant practice to get good result.

Abstract

Head and neck cancers invariably leave a large defect in the face after surgery. Traditionally a large number of local and regional flaps have been used. But the gold standard now has been free flaps. It is very important to know when to use the different free flaps. The most common flaps used are radial forearm flap, lateral arm flap, anteriolateral thigh flap free fibula flap. The cases of recurrence pose a difficult challenge. Where there is earlier neck dissection and radiation. How to go about this while doing a free flap is important. We will be covering the 4 important flaps, the vessels that can be used as recipient, how to monitor the flap.

Abstract

The visibility, vulnerability and social stigmata of facial injury whether by burn, scar or trauma can be compelling for the patient as well as challenging for the Surgeon. Restoration to normal requires tissue replacement which has good colour and texture match and produce minimal visible scarring. Tissue expansion offers an extremely useful alternative which meets almost all criteria of an ideal flap. 92 patients with deformities over various facial subunits were operated and expanders inserted over locoregional areas. Prior planning, accurate measurement and choice of ideal expander is extremely important. Any secondary deformity to adjoining vital structures is best avoided. Results were meticulously and critically analysed. Patient satisfaction was evaluated as well. The presentation underlines the clinical application as well as shortcomings of tissue expansion

Abstract

In the field of plastic surgery liposuction succeeded in becoming the most frequently performed cosmetic surgery procedure during the last decade in both man and women. First liposuction was described with curettage technique and then dry, wet and superwet techniques performed as suction assisted liposuction (SAL). The first aim with liposuction is removing exces fat and making body smaller. On the other hand last ten year everything was changed and liposuction techniques are different due to understanding fat anatomy under the skin. In order to get better results all innovative plastic surgeons need more superficial liposuction with less complications, so more dynamic liposuction or liposculpture techniques was essential. When we want to get shaping on the body we have to do more superficial liposuction. Third generation ultrasound energy (Vaser) is giving 36 Khz sound energy per second and it helps to emulsifiying fat without any damage to surrounding tissues. VASER (vibration amplification of sound energy at resonance) has provided liposculpture techniques to the superficial fat layers and this technique is more applicable for "abdominal etching" with less complication. Vaser is currently the gold standart for use in abdominal etching. Vaser Hi - def surgeries provides less complication and more succesfull results. The technique decision depended on the BMI (it must be <28), degree of skin excess, skin elasticity, patient's sportive history. Between December 2010- August 2016, 221 Patient have undergone vaser liposuction. 89 cases were male and 132 Cases were female. In this cases abdominal vaser hi def procedure was performed In the field of cosmetic surgery to 43 Male patient by same surgeon. All patients satisfied with aesthetic results of their Vaser Hi-Def within the time of follow-up. Seroma rates were %8, unsatisfied definition rates were %1 due to patient Selection, hematoma rates were %1, skin necrosis did not occurred.

Abstract

The cutaneous bacterial microflora is essential in skin homeostasis and is affected by both external (pollutants, cosmetics) and internal (neurohormones) factors which modulate its aggressivity. Nitrogen dioxide (NO2), from automobile traffic, can lead to reduction of the skin protective activity of a commensal bacterium (Pseudomonas fluorescens). Staphylococci are also affected by NO2. Preservatives, such as nisine, modulate the biofilm formation and virulence of cutaneous bacteria. Even sunscreen additives, such as TiO2 nanoparticles, directly influence the growth and surface polarity of P. fluorescens and Staphylococcus aureus. From the inside, skin neurohormones, such as Substance P or Calcitonin-Gene-Related-Peptide (CGRP), diffuse in sweat and form a complex regulatory network controlling skin bacterial virulence The skin microbiote is then a promising target for new cosmetics. The effect of NO2 can be partly antagonized by a cosmetic. Similarly the action of Substance P can be reduced by exposure of the bacteria to a polysaccharide and/or thermal water. These compounds are probably acting through non-specific mechanisms but as bacterial sensors have been characterized it should be possible to design molecules acting as specific antagonists of these factors. However, cosmetics can be also designed to re-equilibrate the microbiote and favour the development of useful bacteria. This can be achieved through the use of prebiotics or probiotics, but also bacteriophages (specific bacterial viruses). Bacteria themselves can produce molecules increasing the skin barrier, as specific non-toxic forms of lipopolysaccharides. Finally, as for pro-drugs, the skin microbiote should be employed to generate short life active products.

Abstract

Fat Pads Baggy, Puffy eylids are viewed by most people as a sign of exhaustion aging. Many individuals will go to great lenghs to improve appearance of their periorbital skin using cosmetics, fillers, chemical peels, lasers and surgical procedures. The blepharoplasty, the fourth most popular surgical cosmetic procedure, as a method to attain a more desirable appearance. However, patients desiree similarly effective yet less invasive solutions with faster recovery times. Injectable phosphatidylcholine at a concentration of 50mg/ml was compound under sterile conditions using a germany manufactory compound named LIPOSTABYL (Aventis Pharmaceuticals ) and Essenctiale (Rhone Poulenc Rorer) 0,5 inch, 30 gauge needle formula PPC 4.95g deoxycholic acid, sodium salt 4,75g benzyl alccohol 0,9ml sterile water for injection 5ml Localized Fat Deposits Subjects with localized fat deposits commonly receive suction lipectomy as a cosmetic procedure. A new office procedure for correction of those superficial fat deposits was applied in 50 patients by injection of Phosphatidylcholine. The method itself consists of using a 30G1/2 insulin needle to inject about 5ml (250mg/5ml) of phosphatidylcholine into the fat, distributing it evenly in a 80 cm square area. Pre and post treatment photographs were taken for technical planning and analysis of the results over the long term. A clear improvement occurred in all, with marked reduction of the fat deposits without recurrence over a two year follow up period and no weight gaining. The injection of Phosphatidylcholine into the fat deposits is a simple office procedure that can sometimes postpone or even substitute surgery and liposuction. Aging Neck The lower third of the face and neck have distinct changes that occur with aging The lost of the perfect jaw line due to laxicity of skin and the drop of fat bichard. The neck looks like a turkey neck. Theses changes can be globally and dramatically addressed with traditional rhytidectomy. However, patients increasingly seek less invasive procedures. Injections with Lipostabil endovena a mixture of phosphatidylcholine and deoxycholate, a bile salt have been used to reduce unwanted, accumulated fat. Recent in vivo investigations indicate that phosphatidylcholine and deoxycholate cause adipocyte lysis, an inflammatory processes and fibrosis. These reactions lead to skin retraction. We seek to report our experience treating aging neck using Lipostabil endovena. Laxity of the skin improved in all patients. A marked reduction in double chins was noted and the jaw line was reconstructed. Adverse effects, including transient burning, erythema and local swelling were present in all patients, but resolved themselves without intervention. Our clinical experience supports the conclusion that Lipostabil endovena treats subcutaneous fat tissue and that fibrosis after the inflammatory process retracts the skin, tightening the area and improving the aging neck.

Abstract

New concepts in mammaplasty: A reliable and producible technique for breast reduction, mastopexy and augmentation mastopexy. Introduction: The mammoplasties are a widely used procedures. Variable techniques are described to help solving some difficult technical situations. This presentation proposes a one reliable and producible technique for breast reduction, mastopexy and augmentation mastopexy. Methods: The new concepts in this technique are: Limited subcutaneous dissection Limited NAC flap Up to down mounting: from NAC to the base Excision of the HERNIATED gland Original adapted skin resection Results: A total of 86 patients underwent this technique (27 breast reduction, 16 mastopexy and 43 augmentation mastopexy) from 2013 to 2015. The average age is 33,75 years (24 to 56 years). The sternal notch to nipple distance is from 19 to 35 cm (range: 24,15 cm). In the cases of augmentation mastopexy all the implants are placed in a sub-muscular position. Complications are observed in 4 cases (4,65%): 3 hypertrophic scar, 1 hematoma and 1 Implant malposition. No wound breakdowns and no complete or partial necrosis either in the areola nor skin. Conclusion: This mammoplasty technique is based in new concepts. It is characterized by vascular safety and skin resection adaptation. It is safe and producible to all the breast mammoplasty situations specially augmentation mastopexy where the skin resection is adapted to the new volume.

Abstract

Rhinoplasty is a complex surgical procedure aiming at attaining a well-functioning and aesthetically pleasant nose. The use of grafts is of the utmost importance for the nasal surgeon to achieve such results. However, the philosophy and technical use of nasal grafts are different in "closed" and "open" rhinoplasty. The aim of the study is to describe the main principles of grafts use in "closed" rhinoplasty derived from our experience in Arabian nose, with special reference to the philosophica This study includes 60 cases. All of them are females aged from 18-45 years. In all the cases I operate using the closed technique with cartilage graft which differs according to each case. Follow up was done up to 1 year i. Patient satisfaction was very good in 50 cases, good in 8 cases , secondary procedures was done in 2 cases. The use of grafts is of the utmost importance for the nasal surgeon to achieve such results. However, the philosophy and technical use of nasal grafts are different in "closed" and "open" rhinoplasty. Also due to the present of thick skin in Arabian population nose the use of cartilage graft is very important and the use of closed technique to avoid the unsatisfactory columellar scars.

Abstract

Chemical peeling is one of the most popular minimally invasive cosmetic procedure in the world. It's a process of causing controlled chemical injury to the skin (partial or complete epidermis with or without dermis) by application of a chemical peeling agent that causes exfoliation of the superficial layers of the skin, leading to removal of superficial lesions followed by regeneration of new epidermal and dermal tissues. Different skins of colour need different agents and different depths to achieve best results.

The ligamentous attachment on face, SMAS anatomy has further helped us to excel in post filler results. I prefer to restore and rejuvenate my entire patient keeping my approach to each of my patient a customized global treatment. Working on all zones of the face helps us to achieve good results. I also use various techniques like towering, dumb bell, anterograde retrograde, linear, bolus, micro bolus multiple puncture to single prick technique. I prefer needle over cannula as I feel more in control with needle n also it's more direct than travelling a distance. Another variation in my technique is multilayer approach to face which I feel gives superior results.
